Whether you’re starting a new business or expanding an existing one, we’re happy you’re here! While you’ll be receiving your business address shortly (if you haven’t already), we do need a few bits of information in order to handle mail on your behalf.
Your Form 1583 will be sent to you in your welcome email to fill out online. Once completed, please submit your photo ID here in order to complete your verification!
Acceptable identification includes: valid driver’s license or state non-driver’s
identification card; armed forces, government, university, or recognized
corporate identification card; passport, alien registration card or certificate of
naturalization; current lease, mortgage or Deed of Trust; voter or vehicle
registration card; or a home or vehicle insurance policy.
Once we have that, you’ll be ready to pick up mail as soon as it arrives!
As a reminder, we will always let you know when mail arrives for you or your business via email.

PS Form 1583 Walkthrough
The PS 1583 Form allows us to receive and handle mail on your behalf. We are required to keep this on file for each member. Please download below!
The downloadable copy will be pre-filled out with our information and highlighted with each field that will need to be filled out by the member.
2. This will be the name of the person for which mail will be received.
3a. Please enter the suite number assigned to you. This should be in the “A-123” format.
5. Please list any names associated with your organization who may receive mail.
6. This will be the name of the applicant.
7a/b/c/d. This is the applicant’s home address.
7e. This is the applicant’s phone number.
9. This should be the name of the company we will be receiving mail on behalf of (if applicable).
10a/b/c/d. This is the company’s business address (if applicable).
10e. This is the company’s phone number (if applicable).
11. This is the type of business. This can be things such as an LLC, Sole Proprietorship, Partnership, etc.
12. If this membership is for a business, please list everyone whose mail will be delivered.
14. If you are obtaining the membership for your business, please list the county, state, and year your business was registered.
16. This will be the applicant’s signature. We will sign on our end once received.
